Duplicate Learner Permit or Full Driving Licence

Duplicate Driving Licence.

If your Driving licence has been lost, destroyed or stolen and you wish to apply for a duplicate licence, your application must be accompanied by:

• Application form (D800), fully completed and also signed, dated and stamped at your local Garda Station.
• Two recent identical passport type photographs (signed at back by applicant)
• Fee of €10.00

If any details are changed you will need to provide proof of this e.g utility bill etc for address, for change of name you will need a marriage certificate , passport or birth certificate.

Important Note:

The forms below cannot be downloaded from the website at this time due to a specific signature requirement which has to be affixed to your licence.They are available at our offices, City Library and Garda stations.
